How it works:
1-choose “buy a form” and provide your credit card information to our processor. (Your credit card data will not be kept nor will your contact information be shared with anyone at anytime.)
2-Fill out the online form at your convenience within a 7 day time frame.
3-Upon completion the results are immediately emailed to the address you provide.
4-You can copy the report and paste it into a Word document for editing or keep the report in it’s email format.
5-Share the data with any professional you believe would benefit from the information (in print or in its digital form), such as your child’s pediatrician or psychologist conducting an evaluation.
