What is it?
An online questionnaire for parents who are concerned their child may have autism and want to talk with their pediatrician about their concerns and/or make sure the psychologist who evaluates them has all the relevant information about their child to make an accurate diagnosis.

Our online questionnaire about a child’s history and autism symptoms, which when completed, clearly summarizes their history in a manner a parent can edit as appropriate and give directly to the pediatrician or examiner.
